Your appointment at Two Minds Clinical Practice is reserved especially for you. We understand that sometimes the unexpected happens, and you will need to cancel your booking. We ask that you please provide as much notice as possible, so the appointment can be offered to someone else in need.

If you provide less than 48 hours’ notice, we reserve the right to charge you a late cancellation fee.

• A cancellation fee of $80 applies if you give less than 48 hours’ notice prior to a scheduled appointment. 

• The full consultation fee will be charged if you fail to attend an appointment without providing any notice.

 

Credit or debit card details are required to confirm your first appointment. As per our Credit Card Authorisation Policy, these are securely collected using our online Intake form and a fee will be charged to your card if you provide less than 48 hours’ notice of a cancellation or do not attend your appointment. The cancellation fee (or failure to attend fee) will be automatically charged to your card on the day of your scheduled appointment, unless other payment methods have been arranged.

If you have an appointment booked for a Monday, we ask that you please notify of any cancellations before 3pm on the
Friday prior.

Medicare rebates are only available for face-to-face, telephone or telehealth services provided. No portion of the cancellation fee is claimable.

We understand that you may not be able to attend an appointment due to COVID-19. If this occurs, a video-link or telephone appointment can be arranged for you. Please contact us if you need to arrange a telehealth appointment.

All outstanding consultation and/or cancellation fees need to be paid prior to any further appointments being booked.
